All samples sent to the laboratory should be accompanied by Waikato sample submission forms. Please take time to fill out the details on the submission forms. They will stay with the sample records in the database, and are available for future reference.

it is also important that you include a copy of the NZ Ministry of Agriculture and Forestry (MAF) permit and paper copies of the sample submission forms in with your samples.
